If you’re looking for a new clinic, you will want to review the clinic's success rates. The success rates can be found through the Society for Assisted Reproductive Technology (SART) website. Be sure to compare the success rates of the clinic to national averages. You can also look up the clinic’s reviews online, or ask friends for suggestions.
One thing to keep in mind when switching clinics is that different clinics may have different protocols and procedures for thawing and using frozen eggs. This is usually not a problem, but it will be important for your new clinic to know how your eggs were frozen.
Ultimately, the decision to use the same clinic or transport your eggs to another clinic is a personal one that should be made based on your individual needs and preferences.
Getting your eggs out of storage
If you’re staying at the same clinic, many clinics choose to store eggs in a long-term storage facility versus at the clinic itself. If that’s the case, the clinic is usually responsible for transporting the eggs back to the clinic and laboratory to create embryos.

How to donate eggs
First things first: if you decide that you want to help intended parents grow a family, you can expect to go through a pretty rigorous screening process. You’ll answer a lot of questions and go through a medical assessment
Typically you can expect the process to look like this:
- Application and Interview: You will share more information about yourself, including background on your health and whether you’ve donated your eggs or been pregnant in the past. We will also gather information about your education and family so we can share this with the intended parents down the line.
- Medical testing: If your initial screening indicates that you are a good candidate for Split, once you match with intended parents, you will be tested for diseases such as HIV and hepatitis.You may be tested for genetic conditions such as cystic fibrosis. Importantly, you will also undergo a psychological evaluation to ensure that you’re participating in the program willingly and that you’re prepared for the process ahead.
- Medications: If you go through with the procedure, you will take injectable medications to stimulate your ovaries to produce more eggs. You’ll make several visits to a fertility clinic over a period of 10 to 14 days so the doctor can perform ultrasounds and monitor your ovaries.
- Egg retrieval: The fertility specialist will use anesthesia and retrieve eggs from your ovaries with a needle and special suction device. This is considered a minor surgical procedure that can be done right at a fertility clinic under light anesthesia. It’s quick too — the whole process can take as little as 30 minutes.
So what happens to my eggs after I donate them?
So what happens to my eggs after the egg retrieval? The eggs you’re donating will now move on to be fertilized and hopefully help grow a new family.
Here’s how your eggs will be used to help intended parents:
- Fertilization via IVF — After eggs are retrieved from your ovaries, an embryologist will use them to start the process of in vitro fertilization — more commonly known as IVF. The fertilization, or insemination process, is performed in a lab and involves adding sperm to the eggs. The fertilized eggs will then develop in the lab and hopefully become embryos, the building block of a developing baby.
- Embryo testing — Some intended parents may opt for preimplantation genetic testing (PGT) to be done on the embryos. This process checks for chromosomal abnormalities such as extra chromosomes or missing chromosomes. This process can also flag genetic abnormalities controlled by a single gene, like cystic fibrosis or BRCA mutation.
- Embryo transfer — If the fertilization process is successful and the embryo continues to divide after fertilization, it’s now ready to be transferred into the uterus of an intended parent or gestational surrogate. This step of IVF is also done in a fertility clinic, where a reproductive endocrinologist will use a catheter to place the embryo — or sometimes multiple embryos — into the uterus.
- Pregnancy — If all goes well, the embryo will implant in the uterus, and your donated eggs will result in a pregnancy for the recipient family! Of course, success will be based on a number of factors here — not just your donor eggs. These factors range from the age of the woman carrying the baby to the quality of the sperm used in the insemination process. CDC data from 2019 (the most recent year for which data is available) found that more than 9,000 babies were born that year with the help of donated eggs.
- Birth — When a child is born, the intended parents will be listed on the birth certificate and be considered the child’s parents in the eyes of the law.
Do egg donors meet the parents?
As you’re considering the Split Program, it’s natural to wonder if you’ll meet the intended parents. You may also be debating the type of relationship, if any, that you might want to have with the intended parents and any potential offspring.
Many egg donor agencies keep donors anonymous — that means they only share details with intended parents about a donor’s genetic history, physical characteristics such as eye color, education, and hobbies. They don’t put the two parties in contact with one another and don’t share the egg donor’s email, phone number, address, or even their real name. In fact, many of them even allow contracts where both parties agree not to reach out to the other.

Maybe you’ve formed a relationship with the intended parents who used your eggs and they want a sibling, or perhaps you want to help more families. Either way, you may want to donate your eggs again — and you can. The American Society for Reproductive Medicine guidelines allow for women to donate eggs up to six times. Know that this decision is entirely up to you.

The number of eggs retrieved per cycle will vary based on your age and other factors. But before we jump right into how an egg becomes fertilized, let’s go over the process that happens before fertilization.
The process of fertilization
It all starts with an egg
Females are born with two ovaries that hold millions of immature eggs which all have hopes of one day becoming an embryo, also known as a zygote. Every cycle, a subset of these eggs are ‘recruited’ to the surface of the ovaries and a single egg is selected to grow and ovulate.
At the time of ovulation during the menstrual cycle, a mature egg leaves the ovary and travels through the fallopian tube where it crosses its fingers (metaphorically) that it will meet its potential match. That match being sperm, of course.
Don’t forget the sperm
Unfortunately, it’s not such an easy task for the sperm to get to the egg. This is a part of natural selection, so only the very best will survive. After ejaculation occurs, the sperm begins to travel along the female genital tract with the end goal being to meet the egg.
As we said, this trip isn’t a simple one. As the various sperm make their way to the egg, they’ll be pushed forward with the help of cervical mucus, but the acidic genital environment and immune cells of the woman can cause some sperm to die. It is up to the strongest spermatozoa to make it to the crown and lining of the egg.
When the sperm meets the egg in the fallopian tubes
This is where the real magic happens. Most sperm try to penetrate the egg, but not all make it through. The strongest sperm will make their way through the ovum, but only the fastest will penetrate the egg and actually fertilize.
When the strongest sperm meets the ovum an acrosomal reaction begins which will release enzymes that will eventually dissolve the outer layer of the egg.
They form an embryo
Now that the egg has found a match, within the next few hours, genetic material from the sperm and egg begin to integrate to create a zygote. This zygote holds a double set of chromosomes which is considered a single-cell embryo.
The embryo travels to the uterus
Now that the zygote has formed, it will then begin to make its way through the fallopian tube and into the uterus. Luckily, the lining of the fallopian tube is covered in cilia which helps direct the embryo to the uterine cavity.
Embryo division
Not only does the zygote have an important job of traveling to the uterus but it also begins to divide numerous times in order to become a multicellular embryo which is also known as a blastocyst. The division period starts immediately upon fertilization and can last for up to 3 days. Beyond day 3, the embryo begins to differentiate into two cell types: cells that will one day become the baby, and those that will turn into placenta. This is called a blastocyst stage embryo (from day 5 to 7 post-fertilization).
Implantation in the uterus
When the blastocyst has been established, the embryo must attach to the uterine wall which typically occurs about 6 days after fertilization and is completed around day 9 or 10. Pregnancy is truly achieved once this attachment occurs.
The adherence to the wall is a crucial step as this is where the embryo will receive its nourishment and oxygen from the mother throughout the development of the pregnancy.
How is the sex of the baby determined?
While you may not find out for a few weeks what the sex of your baby is, you may be surprised to know that this is one of the first defining factors of the embryo during natural fertilization. This is thanks to that one sperm that won the race.
A zygote contains 46 chromosomes, half of which come from the mother and the other half from the father. The zygote receives the 46 chromosomes due to the fact that each sperm cell holds 23 chromosomes, as well as the egg nucleus.
However, the egg and sperm carry different types of chromosomes. While an egg cell contains only X chromosomes, a sperm cell contains either an X or Y chromosome. When both cells split off to create their own zygote, you may end up with a girl by obtaining an X from the egg and an X from the sperm (X+X), OR on the contrary, a Y carrying sperm may merge with the egg creating a boy (X+Y).
Where does fertilization occur during IVF, IUI, ICSI, and PICSI?
If you’re having trouble getting pregnant naturally, your doctor may suggest looking into In Vitro Fertilization (IVF), Intrauterine Insemination (IUI), or Intracytoplasmic Sperm Injection (ICSI) as other methods of conception. However, instead of an egg becoming fertilized within a woman’s body, it occurs in a lab setting.

Why do people freeze their eggs?

Like I mentioned earlier, egg freezing is a way to keep your reproductive options open for later by preserving younger, healthier eggs for future use. - Donor sperm insemination: This method involves inseminating one partner with donor sperm, either at home or via intrauterine insemination (IUI) at a fertility clinic. The chances of IUI working is 5-15% per cycle.
- In vitro fertilization (IVF): IVF is a popular option for lesbian couples. One partner's eggs are retrieved and fertilized with donor sperm in a laboratory setting. The resulting embryos are then transferred into the other partner's uterus for pregnancy in what is called Reciprocal IVF. The chances of IVF working is 30-50% per cycle.
If you choose to go the IVF route, the chances of success depend greatly on the quality of the eggs. As females age, their egg quality and quantity decreases, which can lead to lower rates of successful fertilization, implantation, and live births.
In fact, your age when the eggs are retrieved is far more correlated to IVF success than the age you are when carrying the pregnancy.
What is reciprocal IVF?
Reciprocal IVF, sometimes known as shared motherhood, is a form of IVF specifically designed for lesbian couples (and for some couples where at least one partner is transgender or nonbinary).
Here's how it works: one partner undergoes ovarian stimulation and egg retrieval. The resulting eggs are then fertilized with sperm and transferred to the other partner's uterus for pregnancy.
Since one partner's egg is used and the other partner carries the pregnancy, each partner gets to play a role. This could help both partners feel super involved in the pregnancy and connected to the baby.
How is IVF done for lesbian couples?
IVF for lesbian couples (aka reciprocal IVF) involves several steps:
- Ovarian stimulation. The partner who will provide the eggs undergoes ovarian stimulation, which involves the administration of fertility medications to stimulate the development of multiple eggs.
- Egg retrieval. Once the eggs are mature, they are retrieved from the ovaries using a minimally invasive procedure called transvaginal ultrasound-guided follicle aspiration. The eggs are then either frozen for future use, or fertilized to make embryos.
- Fertilization. The eggs are then fertilized with donor sperm in a laboratory setting. This can be done using standard IVF or intracytoplasmic sperm injection (ICSI), depending on the specific circumstances.
- Genetic testing. Many couples opt to have the embryos genetically tested. This can help rule out embryos that are chromosomally abnormal. Preimplantation genetic screening (PGT) can also tell you the sex of the embryo.
- Embryo transfer: Finally, one or more embryos are transferred into the uterus of the other partner, with the goal of achieving pregnancy.
